My top picks are.

Lost in Space: The Complete Series Blu-Ray set
Any sort of packaging where the discs slide into a pocket. Many of the Fox tv series is like this.
Stranger Things 2: Brand new and the discs aren't held in place by the foamy hubs.

Actually, before I even opened this thread my first thought was Lost in Space: The Complete Series Blu-Ray set!

Horrible flimsy packaging on that set. I can't even really think of many others offhand that are that ill-conceived in design.

I'll go with the Omen Collection on Blu-ray. Terrible outer cover (cheap) and the discs are housed in rubber hubs. Why Fox would do that instead of just a standard Blu-ray case (since I think there are only 4 discs) is beyond me.

I'll second Stranger Things: Season 2.  My discs were loose inside, not sliding around but still not on the foam hubs.

Sliders: The Complete Series.  Discs slide into cardboard pockets, 2 per sheet, multiple sheets in the package.  Dumb as sh!t. 

Avatar: Extended Collector's Edition DVD.  Love the movie, hate the cardboard "drawers" the discs rest in.

Star Trek the Next Generation Complete Series.

41 disks in 2 plastic flip page case.  Way too many disks per case...

Award for worst packaging we get at my shop goes to Smallville Complete Series. 

Haven't gotten one of these sets in yet where almost every disc didn't need to have scratches buffed out.

These sorts of pocket packaging are fine for blurays, but are the absolute worst for DVDs.
If more of us valued food and cheer and song above hoarded gold, it would be a merrier world.

I think you're all wrong...maybe because you haven't seen The Brady Bunch complete series with the shag carpeting. The glue holding the packing together got onto the discs. They slid into cardboard sleeves made to look like a flower so they also scratched easily. I've had cardboard sleeves and hubs that didn't work so the discs slipped around, but how did they manage to get GLUE onto the discs?
