Very odd indeed, although this is an Olive Films release so it's more likely they made a mistake on the cover rather than including an never before seen rated R version.

I would be opposed to this.  Ratings do and have changed over time through the MPAA. 

I would be in favor of a rule amendment to allow the rating on the disc to override the cover, similar to many other aspects of the film (time, regions, etc)..

Looking at the Olive Films web site they list it as PG-13.

No need to change anything, it already does: "The authoritative source for information submitted should be the DVD itself.  Please don't submit content from a third party database, and always verify the specifications printed on the cover.  In both cases, errors abound, so always verify the information directly from the DVD whenever possible."

That is all fine and good as long as there is a rating card on the video. There isn't always. If not it must come from the cover.
