New slip-cover scans with Blu-ray banner included, to correct problem noted by synner_man. Errant line-break in Overview corrected to match cover, also as noted by synner_man. (Thanks for the criticism!)

Edition tag removed: the words "Collector's Edition" do not appear anywhere on the packaging (see scans).

Crew entered per credits on back cover.
