OK, I looked at 'Apes', and four or five others picked at random.  Here's a preliminary crew listing for those three areas based on that.

FX    Spl FX By
        Spl FX Supervisor              Spl FX Co-ordinator
        Spl Visual FX
        Visual FX Supervisor            Visual FX Co-ordinator
        Visual FX By

M/U  Make-up Created By
      Make-up By
      Make-up Supervisor
      Spl Make-up FX

Costumes
      Wardrobe Master/Mistress      (Older films - usually pre-1940)
      Wardrobe By
      Costumes Created By
      Costumes By

SFX and Make-up often list numerous subdivisions with lots of technicians and co-this and co-that, just like Sound, but there is always a supervisor/coordinator listed somewhere in the mix.  Those are usually listed in the opening credits.  On films like LOTR or other big budget epics you'll also see a lot of wardrobe dept subdivisions with tons of worker bees listed, but the main person is almost always listed in the opening credits, especially in older films made prior to WWII.

But, those three areas are among the top Oscar categories, and should definitely be included in the Crew listings.  Most of the better movies wouldn't make it to the screen without the expertise of those three areas.

If you're wanting to put together a list of extra crew roles to ask to add - are we going to differentiate between make up and make up effects? Or do you suggest we lump them together?

I put make-up with make-up, FX or not, because they are generally done by the same group of people.  For instance, the ape heads and such have features that are moved by the SFX people, but the attachment of the prosthetic make-up itself is done by the make-up people so ALL the make-up has the proper look when its on the actor.

The Sound category strikes me for all the detailed roles it gets, as opposite to other categories with too little detail even if more relevant to most users I guess, like Direction (no Assistant Directors!), Writing (no Creators!) and Art (no Costume Designers nor Special Effects!).

Maybe that would be simpler, but I see two problems:

Please no open credits, its already way too much scrolling in TV-series with crew members for every episode. Leave it as it is but remove sound, art department and editor.
