Ia m fine on the iPad, still enough room (I have 32GB, didn't feel like spending the money to get the 64GB version). But it's a good point, I need to start clearing up some space on the iPhone!

Perhaps I missed it somewhere, but if we want to sync over 3G, what port needs to be opened in router?  I'm assuming we're talking port forwarding here...

Yes, all stores is the plan.  As to the port, it's whatever port you select from the Tools->Options->Sharing options screen.

Also, the sync options allow selection of cover image size - here's the selection screen.  The default is "Larger":

Indeed, we'd better select the biggest size images, larger files transfer faster than small ones...

The images are stored as files on the device rather than in the database.  This is so the backup can be incremental.  So the first backup will be slower but after that it's fast.

So if my desktop images are mostly in the Very Large to Huge range when i sync the iPhone image will be reduced to the size i select?  And subsequent syncs will only update newer images (and not try and push those reduced images back to the desktop)?

If so that would be good.  I was fearful of using a huge amount of space holding images on the phone that would be too large to be useful there.

Correct - the iPhone/iPad will only pull images that have changed, and will only push images it has downloaded for those you've added on the device.  Pick the smallest size you'll be happy with on the device.

Rotten Tomatoes/Metacritic question...

According to your original screenshots there are what look like RT & Metacritic buttons.  When pushing them, will they open a link in Safari or will something happen in-app?
